INDIANAPOLIS — An Indianapolis man is wanted for Medicaid fraud after he allegedly received funds for attendant care services he never provided to his mother.

According to court documents obtained by FOX59/CBS4, Dakota Simmons has been preliminarily charged with fraud and theft. Both are level 6 felonies, alleging that he fraudulently billed the Indiana Medicaid Program for false entries of hours worked and inaccurate Daily Visit Notes.

On Jan. 16, the Indiana Attorney General Medicaid Fraud Control Unit received a complaint from the United Healthcare Special Investigations Unit. The investigator alleged in the complaint that Simmons had submitted electronic Daily Visit Verification Notes to a company called Inspire Care & Support Services for services that were rendered between March 10, 2025, and March 24, 2025. The claims said he provided personal care attendant services that “he could not have rendered to his client because the client was in a nursing facility.”

At the time, Simmons was employed by Inspire Care & Support Services and worked as a Home Health Aide after receiving his license from the Indiana Department of Health.

The investigator then conducted interviews and sifted through documentation that appeared to indicate that Simmons provided personal attendant care services to his mother approximately six hours per day and seven days a week at his residence based in Indianapolis.
