In 2012, Christine Martino-Fleming realized there was a major problem at South Bay Medical Center Inc., a Massachusetts mental-health treatment provider where she worked as a staff trainer. Employees treating patients with serious psychological conditions didn’t have proper licenses and were given little oversight, she said.
