COLUMBIA, S.C. (WIS) - A Richland County man is facing insurance fraud charges.
According to the South Carolina Law Enforcement Department (SLED), from July 2022 to January 2023, 38-year-old Jacob Nathaniel Lockhart submitted 10 false claims to Aetna by altering bills and claiming fake medical visits to try and get $6,000.
In January 2023, he allegedly sent 30 more fake claims to Aetna, inflating therapy bills and making up sessions that never happened to try and get over $14,000.
From January 2024 to April 2025, he is accused of submitting 80 additional fake claims to Aetna, including 68 made-up bills, seeking more than $100,000.
In May 2025, he allegedly faked a doctor’s note to try and get $20,000 in travel insurance.
On Wednesday, Lockhart was charged with three counts of Presenting a False Claim for Insurance Payment – Value $10,000 or More and Presenting a False Claim for Insurance Payment – Value More Than $2,000, but Less Than $10,000.
The South Carolina Department of Insurance requested the SLED investigation.
Lockhart has been booked into the Alvin S. Glenn Detention Center.
