SALT LAKE CITY (ABC4) — A woman has been federally indicted after allegedly submitting over $4 Million in false claims to Utah Medicaid.
Jazzamin Tashay Clark, 43, has been charged with eight counts of wire fraud, eight counts of Health Care Fraud and five counts of money laundering.
She is reportedly the registered owner of Creative Minds LLC and Luv Well Care LLC, businesses that were advertised as providing substance use disorder treatment and approved as Utah Medicaid providers.
According to court documents, Clark and a “co-schemer” used the businesses to find Medicaid beneficiaries in Utah who were in need of mental health and substance use disorder services, with individuals being partially drawn from those experiencing homelessness and those referred by probation officers and community health centers.
Clark and the co-schemer allegedly rented homes in Salt Lake County for the use of “sober living homes”, but the homes did not have the equipment necessary to promote sobriety, “with Medicaid beneficiaries left to fend for themselves without monitoring of substance abuse or care and assistance.”
Additionally, they reportedly recruited out-of-state individuals that were unlicensed and unqualified and used their NPI numbers to bill Medicaid for services these individuals did not actually provide.
Without providing healthcare services, Clark and the co-schemer created entries in patient charts that were fraudulent.
