Court documents submitted Tuesday show Thomas D. Miles, 52, is facing a second-degree felony charge for allegedly defrauding an insurance company of nearly $29,000 worth of disability claims.
An investigator with the Utah Insurance Department handled the case, uncovering evidence such as; Daily Living questionnaires submitted by Miles to Prudential Insurance, surveillance footage of Miles appearing to be working at an employer, as well as tax documents and Venmo account statements showing sources of income for Miles.
“(Miles) was examined by a medical doctor, who also watched the surveillance video and opined that (Miles)’ ‘self-reported functionality is not consistent with my examination or the surveillance report.’
“Again, the examinee appears to be very functional during the surveillance videos.”
Court documents say the investigator later interviewed Miles, “who admitted he did work while receiving disability benefits from Prudential (Insurance), albeit only part-time.”
Miles faces one count of insurance fraud.
