A doctor turned whistleblower who exposed the cervical screening scandal in the Southern Health Trust has said he fears oversight and quality assurance of the programme have not "significantly changed".
Screener underperformance in the Southern Trust led to more than 17,000 women having their tests rechecked.
The Southern Trust said it was committed to improving its services and learning from mistakes made.
Erin Harbinson, a mother of four from County Armagh, tragically died of cervical cancer after three of her smear tests were misread in a decade.
It was Dr Michael McKenna who had to tell Erin that mistakes had been made.
Dr McKenna said: "What do you say to a patient if your smear test is missed and actually the service is underperforming and had been for a long time and nothing was done?
"[It was] very difficult to sit down with Erin Harbinson and I know that she found it very difficult.
"I found it very difficult because I couldn't answer any of her questions. There was no investigation done at that time.
"All I could tell her was that this went wrong and it shouldn't have happened. Why it happened? I couldn't give her the answers. I could understand her frustration coming out of that meeting.
